In addition to the routine surgeries and emergency appointments, certain specialist clinics and other services may be provided at specific times or on request. However, most of our long term disease management will be undertaken in our Long term disease clinics or “one stop clinic”.
Long term disease clinic “One Stop Clinic” – Annual Reviews
Long term disease refers to medical conditions that may have an ongoing impact on your health. In particular we use this term to refer to the following conditions:
- High blood pressure (also known as hypertension)*
- Diabetes*
- Asthma
- COPD (Chronic obstructive pulmonary disease, previously termed emphysema or bronchitis)
- CKD (Chronic kidney disease)*
- Mental Health conditions such as schizophrenia or bipolar depression*
- Ischaemic heart disease (previous or current angina or heart attack)*
- Heart Failure
- Stroke or mini-stroke (TIA)*
- Rheumatoid Arthritis*
- Osteoporosis
- Thyroid disease (over or underactive thyroid)*
- Atrial Fibrillation (a condition of irregular heartbeat)
- Peripheral arterial disease (usually causing pain in the legs when you walk)*
If you have one or more of the above conditions, we recommend that you have an annual review. Some of these conditions require monitoring with blood tests or other tests such as breathing assessments or urine tests.
RECOVERY STEPS
We provide advice and treatment for patients suffering from alcohol or substance abuse in partnership with the UNITY.
Antenatal clinics
These are run by the midwife.
Maternity services are provided by all the partners.
Infant immunisation/developmental assessment
Appointments are arranged by the staff at the surgery
Adult and Travel Immunisation
If you require routine immunisations, please make an appointment to see the practice nurse. If you require travel immunisation, please call at the reception for a travel questionnaire that you will need to complete and return at least four weeks before your date of travel. The nurses will assess what immunisation you will require, and will be able to offer further advice on request.
Minor surgery
Most of the doctors are approved to carry out minor surgical procedures such as removal of small lumps and bumps, warts and moles. Some specialise in particular skills such as injection techniques. Ask your doctor for further advice.
Newly registered patients
All newly registered patients will be asked to complete a health questionnaire and have a consultation with the practice nurse within one month of registering. Medical treatment is available from the date of registration.
